We coordinate with health plan personnel and other stakeholders involved in member care, such as providers, to identify candidates for enrollment into OnTrak. We also conduct advanced data mining techniques to identify members with a substance dependence diagnosis and who are high utilizers of health care services. Our analyses show that over 70% of high cost members with substance dependence do not have any substance dependence claims:  they are often not getting the care they need.

These services can be combined with our substance dependence predictive modeling, the only SD predictive model we are aware of in the industry, to proactively address these high cost members. By intervening and pre-empting high-cost utilization, OnTrak can help a health plan save even more time, resources and cost in addressing this difficult population.
