OnTrak identifies high utilizing substance dependent members who will benefit most from intervention, and conducts direct member outreach and enrollment. Our program includes patented and member specific medical and behavioral treatment modalities, care coaching and support for 52 weeks by health professionals, and online tools to reinforce member sobriety.  We address a member’s physical, psychological, emotional, and nutritional needs, coordinate care among service providers, and address the gap that currently exists in the treatment continuum.
